Comprehensive Care Plan by a Registered Nurse

Every client has different needs and is at a different stage when it comes to their care. This is why our registered nurses complete a full in-home assessment and create a custom and individual comprehensive care plan for each client. Our team monitors symptoms, pain levels, and shares progress updates. Your nurse case manager will provide ongoing assessments, address concerns, and update your plan over time as your illness, needs, or wishes change.

Coordinating with Other Healthcare Providers

We can help schedule, coordinate, and manage appointments with other healthcare providers like occupational therapists, your doctor, or registered respiratory therapists.

We will also help facilitate communication and connection between all members of your healthcare team.

Advanced Care Planning

HomeCare Evernest will work closely with the client to discuss their healthcare wishes. As part of the palliative care process, we can help facilitate the conversations necessary for creating an Advance Care Plan and end of life decision making. These are some of the most difficult conversations families will have;. We can help gently guide clients, family members, and friends through the process to alleviate some of the uncertainty that comes at an emotional time. Our team has senior advisers we refer to for in-depth end of life planning and considerations.

Medication Reminders

We will make sure you stay on top of your medications. Managing your medications is important for consistency of care and to prevent unnecessary discomfort.

Household Management

Our caregivers can give you assistance with day-to-day activities so that your home is clean, you have nutritious meals, and your to-do list is done. This includes:

  • Light housekeeping
  • Grocery shopping, meal planning and preparation
  • Errand running and outings

Personal Care

Personal care assistance ensures you get the care needed to be comfortable but that some family members may not be able to assist with. This includes:

  • Bathing, bed baths, or showering assistance

24 Hour Care
