Our care teams emphasize communication with a client’s support system, keeping them informed and seeking input. After every visit, your health care aide shares progress notes with your registered nurse. This ensures any needs from changes in a client’s condition or circumstance can be quickly addressed.

Our nurses are highly involved in every step of a care plan. Often this includes coordinating and communicating with other health professionals such as general physicians, occupational therapists, and physiotherapists. 

This promotes consistency of care and also removes the organizational burden and time commitment from a client’s support system.
