Custom Care Plan from a Registered Nurse

Providing the right care makes a world of difference. Every client and family has differing goals and needs which is why our registered nurses create custom care plans.

Your RN Case Manager will work with your existing health care providers to create a care plan.
We can even help develop a care plan that aligns with the requirements to be discharged from the hospital and ensure proper home care is in place upon your release.
We start with an in-person consultation to develop a personalized plan. Then, your nurse case manager provides ongoing assessments and updates your care plan to meet your changing needs.

Coordinating with Other Health Care Providers

We will work in collaboration with other health care providers to ensure you’re getting the right support and are making progress towards your health care goals and wishes.

Our proactive approach ensures consistent coordinated care and communication throughout our time together.

How do you match a continuing caregiver to a client?

We match a caregiver that will best suit a client’s emotional and physical needs. Personality, experience, specializations of a caregiver can affect the type of therapeutic relationship developed.

Is there a minimum number of hours required for visits?

Our home care services and in facility support can be as flexible as you like. Our visits are often for social interaction, trust and relationship building, so there needs to be a minimum amount of time to get the most out of our service. The minimum hours per visit depends on each client’s needs but the starting time is 3 hours.
